Location: Sacramento, Modesto, Stockton, Dixon
Employment Type: Full Time/Part Time
Reports To: Case Manager/Supervisor/House Lead

Position Summary:
A Direct Support Professional (DSP) provides compassionate, person-centered support to individuals with developmental and/or intellectual disabilities. The DSP fosters independence by assisting individuals in achieving their personal goals and participating fully in their communities.

Key Responsibilities

  1. Person-Centered Support:

    • Support individuals in daily living activities such as personal hygiene, meal preparation, and household tasks.

    • Assist with transportation to appointments, social events, and community activities.

  2. Advocacy and Empowerment:

    • Encourage individuals to make choices and take an active role in their lives.

    • Promote self-advocacy and respect for individual rights.

  3. Skill Building:

    • Support individuals in developing life skills, including communication, money management, and social skills.

    • Provide training on self-care, safety, and community interaction.

  4. Health and Safety:

    • Administer medication and monitor health conditions as directed by healthcare professionals.

    • Ensure a safe and clean environment, addressing any hazards promptly.

  5. Documentation:

    • Maintain accurate and timely documentation of services provided, progress notes, and incident reports.

    • Follow agency and regulatory guidelines for confidentiality and data recording.

  6. Team Collaboration:

    • Work collaboratively with individuals, families, and interdisciplinary teams to provide high-quality care.

    • Participate in team meetings and training sessions to enhance service delivery.

Qualifications

Education and Experience:

  • High School Diploma or GED required.

  • Previous experience in caregiving, support services, or related fields preferred.

Certifications:

  • First Aid and CPR Certification (or willingness to obtain).

  • Medication Administration Certification (as required by state regulations).

Skills and Competencies:

  • Strong interpersonal and communication skills.

  • Empathy, patience, and a commitment to supporting others.

  • Ability to adapt to individuals’ unique needs and preferences.

  • Basic computer skills for documentation and communication.

Physical Requirements:

  • Ability to lift 50 pounds and perform physical tasks such as bending, kneeling, or standing for extended periods.

  • Comfortable working in various environments, including homes, community settings, and outdoors.

Position Summary

Community Anchor Services is seeking a motivated and compassionate Case Manager to join our team. The Case Manager will coordinate and oversee Supported Living Services (SLS) for individuals with developmental disabilities. This role ensures services are person-centered, aligned with individual goals, and compliant with Title 17 regulations. Key Responsibilities

  1. Service Coordination:

    • Develop and implement individualized Person-Centered Plans (PCPs) based on client needs, preferences, and goals.

    • Monitor the delivery of services to ensure they meet client expectations and align with regulatory standards.

  2. Client Support:

    • Advocate for clients to access services, community resources, and benefits.

    • Empower clients to make informed decisions and achieve greater independence.

  3. Collaboration and Communication:

    • Act as a liaison between clients, families, support staff, and external service providers.

    • Facilitate team meetings to discuss progress, address concerns, and adjust services as needed.

  4. Training and Oversight:

    • Provide guidance and training to Direct Support Professionals (DSPs) to implement client plans effectively.

    • Monitor and support DSPs to maintain high standards of care and compliance.

  5. Documentation and Compliance:

    • Maintain accurate records, including case notes, progress reports, and compliance documentation.

    • Ensure all services adhere to Title 17 regulations and organizational policies.

  6. Crisis Management:

    • Respond to emergencies and provide support during crisis situations.

    • Develop and implement risk management plans to ensure client safety.

Qualifications

Education and Experience:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (preferred).

  • Minimum 2 years of experience in case management or related roles, preferably within developmental disabilities services.

Skills and Competencies:

  • Strong knowledge of California Title 17 regulations and person-centered planning.

  • Excellent communication, organizational, and problem-solving skills.

  • Ability to build positive relationships with clients, families, and team members.

  • Proficiency in case management software and basic computer applications.

Certifications:

  • Valid California Driver’s License and reliable transportation.

  • CPR and First Aid Certification (or willingness to obtain).

Physical Requirements:

  • Ability to travel locally for home visits, meetings, and client support.

  • Comfortable working in various settings, including client homes, office environments, and community spaces.
